4/25/2025 Dog approached front of kennel with soft wiggly body and tail wagging. Easy leash. Exited kennel. Light to moderate pull. In yard, dog explored for a few moments. Soft wiggly and tail wagging. Sought attention. Demonstrated sit for treat. Allowed for all petting. Walked back with light to moderate pull. Easy return.

DERM - very pruritic; generalized patchy alopecia, erythema and lichenification. P: 1. Continue treatment with cefpodoxim, ketoconazole and doxycycline as prescribed. 2. Diphenhydramine (30mg capsule) - 1 capsule PO BID x 10 days. nhilde

5/6/2025 05/06/25 08:53 Skin is still erythemic with lichenification. Is malodorous. Dog continues to be pruritic. Continue cefpodoxime 100mg po sid x 7d and ketoconazole 200mg 0.5 tab po sid x 7d. Medical deadline stands.

S/O: Gen - BAR, BCS 5/9, good appetite, solicits attention wiggly at front of kennel EENT - No ocular discharge. No nasal discharge. RESP - No stertor or stridor. Intermittent coughing. No sneezing. Normal respiratory rate and effort. GI - No significant findings. A: - Signs of CIRDC noted upon exam. Likely viral etiology with secondary bacterial component. P: - Start doxycycline 10mg/kg PO q24h x 14d - Recheck at 14 days or sooner if concerns arise

Dog's condition is chronic and she is in need of additional diagnostics and treatments. Will not be undergoing surgery at MCACC. Is not available for PUFA or foster. Place dog on portal, urgent medical NHO. If no outlet by COB on 05/07/25, is at risk for humane euthanasia on 05/08/25. Continue current therapies as prescribed.

Nose clean and free of discharge. External ears are free of discharge/redness. Oral - Mild generalized tartar and gingivitis. H/L/MM - Heart rate and rhythm normal. Grade III left sided heart murmur. Normal bronchovesicular sounds. Normal respiratory rate and effort. No coughing or sneezing. No stertor, no stridor. Mucous membranes pink and moist with CRT<2s. GI - Abdomen soft and non-painful upon palpation, no masses or organomegaly appreciated. UROGEN - Intact female. INTEG - Coat is clean and clear of redness and ectoparasites. NEURO - Normal mentation and posture. M/S - Normal gait. No muscle atrophy or limping appreciated. A: - mild mucoid ocular discharge: r/o secondary to mild entropion vs KCS vs periocular skin disease vs conjunctivitis vs other - heart murmur: r/o secondary to congenital vs degenerative disease P: - BNP 1 strip OU q12h x 7d - continue ketoconazole and cefpodoxime as previously prescribed
