8/30/2025 laying on bed, slowly got up, appeared unstable, neutral body and tail, sniffed presented hand, took treats, leash presented, dog sniffed leash, able to touch when end of leash through door. placing score of "yellow" with "go slow" recommendation based on kennel presentation and notes.

8/28/2025 08/28/25 08:00 BAR, very stoic. Shaking a little BCS 5/9 No oral exam. H/L-WNL. HR 70, RR 30 Ataxic IN hind limbs when rising. Hesitant when walking. Has normal CP in both hind limbs. Scarring present on left tarsus. Will need sedation for more complete orthopedic exam and radiographs. Scheduled for today.

8/28/2025 08/28/25 13:15 Radiographic interpretation: Osteophytes on rim of acetabulum, left hip. Dog appears to have had an old fracture of the capitus right hip with osteophytes forming along head of femur. Several radiodense objects present in colon. Consistent with probable rocks. No concerns for a blockage. DX: old fracture of the left femoral head with subsequent DJD. Will need follow up with a veterinary surgeon to set up a longterm care plan. Dog is not available for PUFA or foster. Place dog on portal urgent medical, NHO. If no outlet by COB on 09/04/25, is at risk for humane euthanasia on 09/05/25. Start gabapentin 300mg po bid x 7d.
