6/1/2026 06/01/26 12:10 Unable to safely vaccinate pet's today, 2 pet's in kennel and hiding together, avoidant, low growling. Will continue to try to make friends and try again tomorrow. jp/brye

5/23/2026 Dog was separated from kennel mate and closed outside for feeding when approached. Dog was laying on kennel floor against wall; full body trembling; wide eyes with dilated pupils; ears up; mouth closed; lip lick. Handler offered treat scatter but dog remained in place; continued trembling with wide eyes as handler offered leash; dog would turn head slightly and then continue focused stare on handler and leash; allowed leash to be draped over head; exited kennel with loose leash; tail tightly tucked; walked to nearby yard. Once in yard handler attached extended leash and dog walked away; followed just behind handler with nose to ground as handler walked to bench and sat. Dog would retreat back to end of yard and urinate; paced around with tail tightly tucked, then defecated in far corner of yard; handler stood up to grab waste bag and dog froze in place hovering over stool; paw raised; wide eyes with furrowed brow; handler walked in the other direction and dog retreated so handler could pick up waste. Dog would pace around yard with tail tucked tightly; trembling and heavy panting with drool; furrowed brow and tight facial tension. Dog would not approach handler for treats or coaxing; would retreat away or freeze if handler tried approaching. Dog allowed handler to pick up leash; walked to exit gate; wary of handler but stood by handler's side; allowed handler to attach clip leash; walked to kennel and entered with no issue; removed leash using clip leash.

5/22/2026 Dog was kenneled outside, tucked into corner, tense, trembling, wide eyed. Handler tossed treats, dog showed no interest. Handler able to reach in and drape leash over head, dog tolerated leashing, stayed tucked into the corner for a moment then slowly walked out of kennel, low tail, furrowed brow, did not want to walk toward smaller yard but easily walked toward other larger yard, allowed handler to adjust stopper while sniffing a bush, once in yard dog paced along fence with a tucked tail, V shaped lip, some panting. Handler sat on bench, called to dog, did not approach. Handler gave dog a few minutes to decompress, handler started to approach dog, dog shifted away, handler knelt in yard and spoke softly, dog remained at about 10 feet away, did not come closer. Dog continued to avoid handler in yard, tucked tail, cautious, scanning environment. Handler opened catch pen, dog entered, backed away when handler reached for leash on ground, exited the yard, handler reached to adjust stopper and dog quickly turned head with wide eyes, no attempt to snap, handler walked dog around and allowed them to sniff the ground and nearby bushes, adjusted stopper and placed clip leash hwile dog was distracted, dog easily entered kennel, moved toward back of kennel, allowed handler to remove leash with clip leash.

5/21/2026 Handler approached kennel back; Dog was locked outside for AM cleaning; Handler pulled this dog and her Kennel mate (A5165228) out for the evaluation; This Dog was sitting in corner with unsure stare; ears alert; neutral body; timid; hesitant head duck on approach; Handler spoke softly and unlocked door; Assistant handler already was able to leash kennel mate and walked out of kennel; This dog stood up and went to follow kennel mate out of kennel; Handler was able to stick arm in and leash on first try; Dog walked out with mild to loose leash pull; Followed behind kennel mate then walked into yard first; Dog seems to recall name; Handler adjusted leash slowly and dropped leash; Dog allowed leash adjustment; Dog walked around yard exploring; would follow kennel mate around; shortly later defecating; would scan yard and stand near fence line; Handlers would try to verbally coax over and dog wouldn't approach on own doing; Handler would pet gently if dog was in walk by; Dog was timid with touch; Wary of handler; handler would solicit pets and attention to kennel mate and this dog would be near by but didn't solicit anything; This dog did stand up on fence line and got down shortly later; dog was walking around neutral bodied; timid; tail untucked; just relaxed down; when time to go handler grabbed leash and walked over to exit; Dog was willing and followed; Walked in building and had slight hesitancy when seeing kennel mate not following behind; handler used slight leash direction and dog proceeded to walk into kennel; allowed brief petting into kennel; allowed slow leash removal by hand; stood stoic as handler removed;

5/18/2026 Handler apporached K9 and K9 started to growl and lunge toward handler. Owner walked K9 into intake room. K9 placed self into corner facing handlers and would low growl when handler attempted interaction. K9 came toward handlers when handlers were attempting to vaccinate the other dog K9 came in with. K9 walked to ramp. K9 started to low growl and air snap at handler. Handler used control device to administer vaccines. K9 walked to kennel and clip lead was used to remove leash.

6/1/2026 VET TECHNICIAN EXAM (Date: 06/01/26 10:21) DUE TO TECH MONITOR APPETITE/ ATTITUDE/VOMITING AND STOOL Attitude: QAR-timid BCS: 5/9 Ocular Discharge / Squinting: None observed at this time Coughing/Sneezing: None observed at this time Nasal Discharge - Mucoid / Serous: None observed at this time Vomiting/Diarrhea: None observed at this time(No stool) Appetite: Good-wet/dry adult dog food mixture Great-hot dogs Other Notes of Concern / Observations: Pet in front part of kennel(pet usually hiding in back part of kennel), did slowly run to back when I approached, able to soflty talk to pet but just peeked head thru guillotine. jp

5/31/2026 05/31/26 12:14 Targeted exam - did not remove from kennel P is BAR; scared/shaking and avoidant at back of kennel; BCS 4.5/9 EENT: OU clear with no ocular discharge or blepharospasm. Nose clean with no nasal discharge Cardio/Resp: Normal respiratory rate and effort observed GI: Poor appetite for kibble; formed stool score 3. No vomitus observed at time of exam Derm: Pink and blue dye noted on ventral chest. No obvious skin lesions observed from visual exam Neuro: Normal mentation Musc/Skel: No obvious abnormalities noted from visual assessment, P sat in back corner at time of exam A: - Inappetence for kibble – suspect stress-related anorexia vs diet preference - Reported bile vomiting, not observed on exam P: - Add canned food to daily meals to encourage appetite - Monitor appetite, attitude, vomiting, and stool for next 2 days - Recheck if vomiting persists, appetite declines further, or other clinical signs develop

5/18/2026 Nobivac 3-Rabies, manufactured by Zoetis, Serial number: 838296 administered SQ, R lateral hip, by SG, supervising veterinarian ST. Nobivac Canine 1-DAPPv, manufactured by Merck, Serial number: 02121B31A administered SQ, L Hind Leg. -ABM/SG Nobivac Intra-Trac3, manufactured by Merck, Serial number: 00541523B administered IN. -ABM Unable to complete spay/neuter check due to safety. Bravecto (fluralaner) for dogs >44-88lb (1000mg) 1 tube of topical solution applied in multiple spots between shoulder blades and along back by ABM/SG. Reapply after three months. Microchip scanned in lobby and added to file. Mylar placed in kennel sleeve due to behavior.
