Radiographs: displaced fractures of the proximal phalanges of digits 2 and 3 with associated surrounding soft tissue swelling. Fracture margins are smooth and corticated, consistent with healing/subacute fractures; mild angular deviation and callus formation noted. P: - Dog is not suitable to PUFA or foster at this time. Dog needs transfer to an urgent medical NHO Partner for orthopedic evaluation and management of healing fractures involving digits 2 and 3 of the left hind paw. Follow-up diagnostics, repeat radiographs, activity restriction, pain management, and surgical consultation may be required depending on progression of healing. Dog is at risk of humane euthanasia on 6/11/26 if no outlet is found by COB on 6/10/26, but may be humanely euthanized earlier if QOL declines.

All adult dentition present except canine teeth; age consistent with ~5-5.5 months Cardio/Resp: Normal heart rate and rhythm. No murmur auscultated; Eupneic, normal bronchovesicular lung sounds auscultated in bilateral lung fields GI: Soft, nonpainful abdomen on palpation; great appetite; formed stool score 2-3 Derm: Tick infestation present.
